Make a note of any workshop you want to attend, then come back to this page to sign up. After filling out this page, you'll be taken to a second page for payment.
IMPORTANT: THERE ARE TWO STEPS TO SIGNING UP FOR A WORKSHOP.
Step 1) Fill out the form and press submit.
Step 2) Pay separately via credit card, check, or money order. Step 2 is on the next page.
STEP 1: FILL IN FORM AND SEND. Please set your cursor inside the text boxes and enter your name, address, etc. Click on the Submit button to send your data directly to Douglas Mehrens at EAI.
Workshop Participation Conditions:
To reserve your space in the workshop, please remit a deposit of $75 (for two-day workshops) or $100 (for three-day workshops) as applicable. Please note the Institute’s cancellation policy is as follows: All deposits and fees are payable to the Encaustic Art Institute via credit card or Pay Pal, or by mailed check to: 18 County Road 55 A (General Goodwin Road), Cerrillos, New Mexico 87010. Final payment is due two weeks prior to the start of the workshop. If a workshop participant cancels his/her participation for any reason whatsoever, the Institute will retain the participant’s deposit. The prospective participant may apply the deposit to another Institute workshop, provided application is within 12 months of the cancellation. If the Institute cancels a workshop for any reason whatsoever, all deposits and fees will be fully refunded to the registered participants. The Institute assumes no liability for any injuries, thefts, losses, or illnesses incurred by participants during or as a result of participating in a workshop held on the Institute’s premises. All worskhop participants will be asked to sign and return a liability waiver prior to workshop participation. The signed waiver is due the same date as final payment isdue (two weeks prior to the workshop’s start date).
